The shape and fitting of the flat top pieces.
The drawer handle - wood, brass, iron?
The type of door.
The door handle.
The door can be hinged on the left or right.
The door hinges and catch - brass, steel?
A lock could be fitted, to the drawer or door.
The internal shelf could be omitted or modified.
The decorative cut edge of the bracket feet could be
reshaped.
The wood. If not pine then oak is popular.

Or maybe...

A hidden compartment, for the storage of secrets.
An opening in the back of the cabinet to allow for an electrical cable.
